Summer 2022 Registration

I/We, the parent(s)/guardian(s) of the above-named player(s), know that participation in baseball may result in serious injuries and that protective equipment does not prevent all injuries to players. I/We hereby waive, release, absolve, indemnify, and agree to hold harmless Crackerjacks Baseball, its owners, directors, organizers, umpires, and participants for any claim arising out of injury to my child or children named above, whether the result of negligence or any other cause, except to the extent and in the amount covered by accident or liability insurance. Further, I/We hereby grant permission to the adult director or organizer of Crackerjacks Baseball Club to obtain emergency medical care, at my/our expense, from any licensed physician, hospital or medical clinic, for the player(s) named herein at such time I/We cannot be contacted in person or by telephone. The authorization shall include all organization activities in the 2022 calendar year, including travel to and from all league activities.

I/We, the undersigned, have read the Crackerjacks Baseball "Rules" posted on the website. I/We agree to follow these rules and to review the rules with my/our player(s).

I/We, the undersigned, understand that my/our player(s) will be under my/our care at all times during Crackerjacks Baseball activities, and I/we agree to supervise my/our player(s) during such activities.

I/We, the undersigned, understand that in the event of bad/inclement weather, Crackerjacks Baseball will either reschedule the game or refund me/us a pro rata amount for a game it is unable to reschedule. In the event the game is rescheduled, I/We understand that there is no guarantee it will be at a time my/our player(s) can attend. I/We understand that this unavailability will not entitle me to the pro rata refund.

I/We also hereby give permission to Crackerjacks Baseball, and/or parties designated by Crackerjacks Baseball Club to photograph and videotape my/our child/children and to use such photographs and videotapes in all forms of media, for any purposes including advertising, audiovisual exhibition, television commercials, community service announcements, press advertisements, print publications, public relations displays, training manuals, strategic plans, social media posting, posters and promotional displays in public venues. I/We further consent to the use of my child’s/children’s name in connection with the photographs or videotape on the league website or social media sites. I/We understand that there will be no financial compensation for the use of the photographs, videotape, and/or the name of my child/children. I/We understand that this consent shall remain valid until I/we withdraw my consent in writing and I hereby release Crackerjacks Baseball from any and all claims that may arise out of the use of the photographs, videotape and/or name of the person named below. I/We also grant Crackerjacks Baseball the right to exclude 3rd party photographers from the photographing or videotaping my/our child/children on any premise owned, leased or rented by Crackerjacks Baseball.

For safety reasons, Crackerjacks Baseball scrimmage games are not “drop-off” baseball activities. Each player must have a parent/guardian present for the entirety of each scrimmage. No player may be on the field unless his or her parent/guardian is present at the field (not "on" the field). If this rule is found to be violated, the player may lose his or her membership in the Club.

Parents/guardians may be asked to supervise a dugout during scrimmages.
Parents/guardians are not coaches and are not to be managing the team, pushing team strategy, coaching runners, or instructing players on ball-playing. Dugout parents are there only to keep the peace, ensure the safety of the players, and to assist with equipment and logistics of field transitions.

For insurance purposes, Crackerjacks Baseball Club must perform background checks on all volunteers (paid or unpaid) as well as independent contractors who may be in contact with children.
